Evidence of U.S. Citizenship: Examples of Primary Citizenship Evidence Born in the United States? Proof of Identity: Washington State ID, Driver's license, Washington State School ID (Students); a secondary ID is required if providing out of State ID.
One 2x2 color photo with a white background: Photo Basics: bring your own color photo, taken in last 6 months Use a clear image of your face. Available at Rite Aid, CVS, Walgreens, Bartell Drugs, etc. Do not use filters commonly used on social media. No selfies. Take off your eyeglasses for your photo. Required for Children age 15 & under: Will be accompanied by both parents. All applicants must be present.

Both parents are required to be present with the child(ren) as a safety measure per regulation.

PAYMENT INSTRUCTIONS:

One check/cashier's check/money order made to U.S. Dept. of State. (Adult Passport $130 USD, Minor Passport $100, Expediate fee +$60, Passport card +$30, Passport card (minor) +$15 [effective 12/27/21])
Second payment made to City of Mill Creek via check, debit, credit card accepted (NO CASH). ($35 per application)

Processing Times
According to U.S. Department of State, processing times for routine and expedited applications are as follows:

Up to 18 weeks for routine processing
Up to 12 weeks for expedited processing
Mail times vary and delays have been significant. Plan ahead.
